St John's wort affects the way many prescription medicines work including the oral contraceptive pill; consult your doctor.
Prescribing Information:
Avoid using in pregnancy and lactation.
Concomitant use of St John's wort and drugs that elevate serotonin levels, such as tricyclic antidepressants or SSRIs, may result in additive or synergistic effects and increase the risk of serotonergic syndrome.
As the constituent hyperforin (in St John's wort) has a significant and selective induction effect on CYP3A4 and 2C19 activity, interactions are possible with those drugs that are substrates for CYP3A4 or 2C19 and/or rely on P-gp transport.
Breakthrough bleeding has been reported with use of St John's wort, which can indicate decreased effectiveness of oral contraceptives.
Saffron is not recommended in bipolar states.
Eleutherosides, particularly B and E, appear to inhibit the activity of CYP2C9 and CYP2E1 and may inhibit the metabolism of drugs metabolised by these pathways. Therefore, Siberian ginseng may falsely elevate assays for blood digoxin; increase the side effects of phenelzine and other MAOIs; increase the risk
of bleeding in patients taking warfarin; and increase tolerance for chemotherapy and improve immune response.
Since studies suggest that the effects of Siberian ginseng attenuate after four weeks of use, consideration of breaking treatment for two weeks after this period before resuming is clinically recommended in elderly patients with hypertension.
